CIO Americas conducted a survey every year to measure its customer satisfaction. The survey was usualy an excel / access formular that passed along via email. For the year 2004 CIO Americas decided to move the survey online which offers them more advantages. I developed the survey using PHP and MySql, on the top of it there are CSS and XHTML. The whole project also includes the result view for that survey.
